New clients can schedule their first appointment by phone, email or text or by emailing this form to denise@lifequestaz.com. Once scheduled, you will be given access to our secure patient portal where you can complete our client forms electronically.

Initial Family Intake Session - $250.00
Ongoing Family Sessions - $225.00
Couples Counseling
Initial Couple’s Intake Session - $225.00
Ongoing Couple’s Sessions - $200.00
All sessions for Seniors (age 60+), Military Veterans, and First Responders: $170.00
Individual Counseling
Initial Individual Intake Session - $180.00
Ongoing Individual Sessions - $150.00
All sessions for Seniors (age 60+), Military Veterans, and First Responders: $130.00
